Custom icons for form/query headers

There has been some issues with this configuration in pop up forms and there is another script for pop up forms:

parser.m_iconCls = "MyIcon";

Thanks to Awaresoft support.

NB: This configuration only lets us control the pop up form icon and not the other things e.g. extjs tools, align title etc.
