It appears that you cannot use the same icon on more than one section of a form. If you do only one section displays the icon, the others are blank.

As sections have applicability it's quite right to want to display the same icon on more than one section where those sections are basically the same.

As a workaround I have just made copies of the required icon file and given them unique names.

if you can't fix the issue then this at least gives us all what we need.
