FR - New Application Default Settings

Creating a new BSV should allow for preferred defaults.

1. Common Application
2. Separate Database
3. Separate Testing Database

with 2 above defaulting to the title of the Application and 3 above defaulting to database in 2 with Test appended.

I never use the common database, because I like the separation of data that individual databases provide.
